Support and advice for rough sleepers, where they are, and on their terms

What is Rough Sleeper Outreach?

The Enhanced Rough Sleeper Outreach team regularly monitor the local area, checking known and recently highlighted locations for people sleeping rough, or where there is evidence of rough sleeping.

The Rough Sleeper Outreach team is made up of different members from across our multi-agency team.

The Rough Sleeper Outreach team is the first part of a connected journey for those rough sleeping

  • The Outreach team engage and conduct initial identification checks

  • Rough sleeper navigators build relationships and conduct welfare support

  • Vulnerable person support workers based at the centre are here to listen, provide guidance and signpost to:

    • Immediate needs - food, clothing

    • Housing team

    • Financial advice

    • Medical support

  • Rough sleeper prevention workers offer support and take early action to help prevent vulnerable individuals from sleeping rough.

  • Complex needs and Housing support workers provide ongoing support for those on their journey out of rough sleeping
