Some of these partners work closely with us on a day to day basis at the centre, while others support us and work behind the scenes. North Devon Council recognised that the needs of some of our most vulnerable residents would be met more effectively through a collaborative, multi-agency approach. Those who find themselves homeless or in insecure accommodation often face difficulty in accessing the support and services to progress into stable and healthy housing. This spurred an initiative to integrate service provision and provide wrap-around support for those with housing need. Working in partnership with the existing charity, the Freedom Community Alliance, the council created an enhanced rough sleeper service, expanding its capacity to exercise the non-statutory power to accommodate, and provide a robust prevention and recovery service to combat homelessness with the team being collocated at the Freedom Centre.
Inclusion Health Devon CIC delivers the Inclusion Health service from the Freedom Centre in Barnstaple. The clinical team of Louise Scantlebury, Inclusion Health Nurse, working alongside Dr Nigel Moody, who provides a weekly GP session each Thursday. Together, they support people who are experiencing homelessness or insecure housing to access timely, compassionate primary care in a setting they already know and trust.
The service has been based at the Freedom Centre since 2021 and has evolved into the commissioned model in place today. It is commissioned by Devon Integrated Care Board and depends on strong partnership working. Day-to-day collaboration with Freedom Community Alliance is central to the model, alongside joint working with North Devon District Council (NDDC), Devon Partnership NHS Trust (DPT), Waythrough, and specialist secondary care services. This joined-up approach helps coordinate clinical care with practical support around housing, benefits, mental health and substance misuse, enabling people to access the right help at the right time and to stay engaged with services over time.

Having a Together role within the Freedom Centre has proven to be highly valuable for both clients and staff. This role enables early intervention, which is vital in supporting individuals at the beginning of their recovery journey and preventing challenges from escalating. By being positioned within a 'one‑shop' environment, the role allows for efficient and timely signposting to a range of agencies and specialist services. This ensures that clients receive the right support at the right time, without unnecessary delays or barriers.
Working collaboratively within the wider multidisciplinary team also strengthens communication and promotes a clearer, more unified approach to client care. Staff benefit from shared knowledge and a deeper understanding of each client’s needs, while clients experience a more streamlined and supportive pathway. Overall, the Together role enhances engagement, improves outcomes, and contributes to a more responsive and holistic service within the Freedom Centre.
